Vacancy 105/2020: Intake/Follow Up Coordinator, Te Hononga Hou Mental Health & Addictions services – Bay of Islands area – (1.0 Fte)
We are looking for a dynamic person to provide ‘Intake and Follow up’ support services for Te Hononga Hou Mental Health & Addiction services within the Ngati Hine Health Trust.
This full time role (40 hours per week) will work with the SPOE team (Single Point of Entry), referrers, practitioners completing assessments, detox services, Corrections, AOD and MH services and other key stakeholders. Tangata and their whanau (with consent) will be involved throughout the care pathway from pre entry through to follow up.
The Intake/Follow up Coordinator is also responsible for supporting evaluation collection and data analysis.
